Introduction
This course is designed to cover the essential elements for all safety, operations, engineering and managerial personnel with the responsibility for carrying out and participating in HAZOP.
The course will introduce the participants on how to use and master the HAZOP methodology, a study team technique, to
- Identify potentially hazardous failures of a process with up-to-date P&IDs in a systematic, thorough and creative way using guidewords
- Determine the causes that cause the process to operate beyond the design envelope,
- Evaluate the safeguards and perform a risk assessment to ensure that appropriate risk mitigation measures are provided.
- Adapt, extend the HAZOP methodology concept and apply it to cover What if/Checklist study for work, storage and transportation activities, batch processes taking human factors into consideration, equipment Failure Mode Effect Analysis (FMEA) and Computer Process Control (CHAZOP)
- Conduct HAZOP revalidation study
HAZOP is used throughout the project development. It normally revalidates every 5 years throughout the life of petrochemical chemical plants, pharmaceutical facilities, onshore and offshore production facilities, semiconductor facilities.
At the end of this course, learners will:
- Understand and master the HAZOP Methodology
- Understand Risk Ranking and Risk Matrices for use in the study
- How to select and markup HAZOP nodes
- Learn how to document the HAZOP findings with all required details such that the HAZOP report can be used as inputs for SIL classification study, Alarm rationalization study and emergency response planning
- Understand and follow the HAZOP workshop ground rules
- Understand the duties and responsivities of facilitator, scribe and team members
- Be familiar with some common errors committed in conducting HAZOP study
- How to plan and prepare so that A HAZOP study is executed efficiently
- How to revalidate existing HAZOP studies without doing a complete re-HAZOP
- Understand the critical importance of good facilitating skills to the success of HAZOP
- Understand the relationship between HAZOPS and the other elements of a Safety Management System
- HAZOPS methodology
- Differences between HAZOPS and other reviews, including design reviews
- Understand the steps that must be taken to prepare, conduct, report and follow-up on a HAZOPS
- Common errors committed by inexperienced HAZOPS leaders
- Developing good HAZOPS facilitating skills
- Interactive HAZOPS Workshop practice

Requirements: Basic process engineering knowledge. Able to read and understand process flow diagrams (PFD), and piping and instrumentation diagrams (P&IDs).
