Short Leave Application (MC) for MAE student

• If you are unwell and have a Medical Certificate (MC), please complete and submit the e-form here within 7 working days from the date of your medical leave. Late submissions will not be accepted. 
• Note: The e-form is applicable to MAE undergraduate students (both full-time and part-time) only. Non-MAE students should not use this form and are advised to submit their medical certificates (MCs) to their respective schools.

Important:

Do not use this e-form for the following types of leave, as different procedures apply:

• Medical leave taken during examination periods.
• Leave taken during attachment/ internship. Please apply via the online internship system, InPlace. The Career & Attachment Office (CAO) will inform you of the outcome.
