Living Well with Hypertension and Hypercholesterolemia

 
This course is a formal training program aimed at enhancing the competencies of allied healthcare professionals, nurses, and other health-related workers in the care of elderly individuals with chronic diseases, such as hypertension and hypercholesterolemia. The need for this course stems from the lack of formal training programs available to upskill these healthcare workers in the management of these chronic conditions.

The course is aligned with recent healthcare initiatives, such as "Healthier SG," which emphasizes community-based and lifestyle-oriented healthcare delivery, and the "National Plans for Successful Ageing," which aims to increase the number of Senior Activity Centres to support the social and health needs of the elderly.

At the end of the course, learners will be able to:

  • Coach and counsel patients (mostly senior citizens) on the prevention and management of hypertension and hypercholesterolemia through behavioural and lifestyle strategies such as behavioural change, overcoming substance abuse e.g., smoking and alcohol, and in exercise and diet management.
  • Manage community and healthcare treatments in specialist and community clinics, and in Senior Activity Centres.
  • Design treatment plans for patients and senior citizens on lifestyle and behavourial interventions.

The course curriculum focuses on empowering learners to effectively educate and guide patients in managing their health conditions through assessments, monitoring, and the implementation of medication and lifestyle strategies. Learners will also acquire practical skills in lifestyle modification strategies, exercise and dietary management, and the use of publicly available resources to plan their meals and exercise routines.

This course is a valuable investment in upskilling the workforce in the care economy, providing them with the knowledge and skills necessary to effectively support elderly individuals with chronic diseases. 

This course is suitable for nurses, Allied health professionals (AHPs), healthcare workers in Senior Activity Centres, and other health-related workers e.g., health coaches.

Dr Muhammad Ilman Bin Mohd Adil

Dr Ilman graduated from NUS Medicine in 2012 and has been an accredited Family Physician since 2020. He earned his Graduate Diploma in Family Medicine from NUS in 2018 and Graduate Diploma in Sports Medicine from NTU in 2021. He is a strong believer in a holistic approach to lifestyle-related chronic diseases with an emphasis on exercise. He earned the NCSF Personal Trainer certificate in 2018. Currently, he is working with Thomson Medical and in his spare time, you can find him in the gym, running around the neighbourhood, or spending quality time with his family.
