Living Well with Diabetes Mellitus

 
This course is designed to enhance the skills and competencies of allied healthcare professionals, nurses, and other health-related workers who provide care for the elderly patients with diabetes. The goal is to equip these healthcare professionals with the knowledge and practical skills necessary to effectively coach and counsel patients on the prevention and management of diabetes through lifestyle and behavioural changes.

Upon completion of this course, participants will have gained a thorough understanding of the comprehensive management of diabetes, encompassing the critical role of exercise and nutrition in disease management. Armed with this knowledge and practical skills, they will be better equipped to deliver effective and patient-centred care, positively impacting the health and overall well-being of those in their care.

At the end of the course, learners will be able to:

  • Acquire and retain accurate basic knowledge of exercising with diabetes
  • Identify the safety aspects of each exercise
  • Explain how to carry out each exercise safely
  • Develop a personalised plan/ regime to allow the diabetic patient to incorporate regular exercise and live a healthier lifestyle
  • Acquire the necessary skills sets in managing community and healthcare treatments in specialist and community clinics, and in Senior Activity Centres.
  • Enable participants to acquire the necessary skills to come up with treatment plans for patients and senior citizens on lifestyle and behavioural interventions.

The course curriculum focuses on the pathophysiology of diabetes and provides structured training on exercise and nutrition programs that can be implemented in patient homecare settings. Participants will gain hands-on experience in developing treatment plans that incorporate lifestyle and behavioural interventions, as well as practical skills in managing patients in specialist, community clinics and senior activity centres.

This course is suitable for nurses, Allied health professionals (AHPs), healthcare workers in Senior Activity Centres, and other health-related workers e.g., health coaches.

Dr William Chan

Dr William Chan has a Masters in Family Medicine from National University Singapore (NUS) and a Graduate Diploma in Sports Medicine from NTU, Singapore. He has 14 years of experience working in various hospital and clinic settings, including in the emergency room. He is currently running his own home medical service called Care At Home. He teaches post-graduate family medicine with the College of Family Physicians (CFPS). He also teaches undergraduates at the LKC school of medicine. The Living Well series will help healthcare professionals and stakeholders to better equip themselves with the skills and knowledge to understand and manage people with diabetes and its related acute and chronic complications based on a person-centered approach. Exercise is one of the best examples of a useful preventive health measure to prevent organ complications from diabetes mellitus. This is often a simple way of managing diabetes with fewer prescriptive medications. It is also one of the reasons why Healthier SG and various efforts by community partners support a holistic approach towards living well. We hope participants will learn concrete tools to support patients in their journey towards better health.
