​SEN Fund Application​ Form
If you wish to apply for the SEN Fund, please download the SEN Fund application form, complete and email the service request form to aeu@ntu.edu.sg​​. Please verify your eligibility for SEN Funding by visiting url: http://www.ntu.edu.sg/SAO/OurServices/SWII/AccessibleEducation/Pages/SEN-Funding.aspx.

Academic Assessment Accommodation Application Form

If you wish to apply for academic assessment accommodation, please follow the application process below. 

  1. Download the Application Form for Academic Assessment Accommodation.
  2. Complete the form and submit it together with supporting document to AEU at email: aeu@ntu.edu.sg
  3. Arrange for meeting with AEU to discuss the application and needs before week 2 of the semester.
  4. Provide original supporting document indicating permanent disabilities/ SEN and needs for special examination accommodations for validation by AEU.
  5. Approvals on academic assessment accommodations will apply to all types of academic assessments in NTU i.e. CA, quizzes and mid-term tests and final examinations.
  6. Approvals on academic assessment accommodations will apply throughout the students' candidature at NTU.

 

Supporting documents may include the following:

  • Documents indicating academic assessment accommodations provided at national examinations i.e. 'O' level, 'A' level, International Baccalaureate (IB) or from immediate past educational institutions (e.g. polytechnics).

 

  • Assessment or diagnosis must be conducted by qualified professionals who are trained to assess needs of persons with disabilities and SEN (e.g. psychiatrists, psychologists, occupational therapists).

 

  • All assessment reports indicating recommendations for academic assessment accommodations must not be dated more than 3 years from the date of academic assessment accommodations application to the University.

 

  • All assessment or diagnostic reports must indicate clearly students' disabilities or SEN.
