Group Personal Accident Insurance 

Benefits

1A. Accidental Death - 100% of Capital Sum Insured of S$10,000 per Insured Person

1B. Accidental Permanent Disablement - up to 150% of Capital Sum Insured of S$10,000 per Insured Person

2. Accidental Medical Reimbursement (AMR) - S$3,000 per Accident involving hospitalisation / surgery; for claims without hospitalisation / surgery, AMR will be sub-limited to S$1,000 (per Accident)

Definitions

Accident or Accidental means a sudden, unforeseen and fortuitous event that results in the Insured Person suffering Death, Disablement or Bodily Injury.

Insured Person shall mean the Insured Students of NTU.

Injury shall mean Death, Disablement or Bodily Injury which is sustained by an Insured Person during the period of Insurance and is caused by an Accident solely and independently of any other causes within 365 days from the date of such Accident.

Permanent shall mean lasting 365 days from the date of Accident and at the expiry of that period being beyond hope of improvement.

Disablement means each of, or any of, Permanent Total Disablement, Temporary Total Disablement or Temporary Partial Disablement that results solely, directly and independently of all other causes from Bodily Injury and which occurs within 365 days of the Accident in which such Bodily Injury was sustained.

Accidental Medical Reimbursement shall mean the reimbursement to the Insured Person the Usual Customary and Reasonable Medically Necessary Expenses as defined, incurred up the benefit level stated for Injury.

Medically Necessary Expenses shall mean expenses incurred within 365 days of sustaining Injury paid by the Insured Person to a legally qualified medical practitioner, physician, surgeon, nurse, hospital and/or ambulance service for medical, surgical, X-ray, Hospital or nursing treatment including the cost of medical supplies and ambulance hire but excluding the cost of dental treatment unless such treatment is necessarily incurred to restore sound and natural teeth and is caused by Injury.   All treatment must be prescribed by a qualified medical practitioner in order for expenses to be reimbursed.

Usual, Customary and Reasonable Medically Necessary Expenses shall mean a charge which is made for treatment, supplies or medical services medically necessary to treat the Insured Person's condition; does not exceed the usual level of charges for similar treatment, supplies or medical services in the locality where the expense is incurred; and does not include charges that would not have been made if no insurance existed.

Geographical limit

Geographical limit: 24-hour world-wide coverage, except for part-time undergraduates, whose cover shall be limited to Accident in consequence of the following:

a. In the premises or on the grounds of the University while attending lessons or workshops, or

b. Whilst travelling between the undergraduate's residence / employer's (if working) premises and the University to attend lessons or training conducted in the University.

1. Subject to terms and conditions of the Insurance Policy.

Eligibility

(1) Without prejudice to Item (3) below, the Policy shall cover all matriculated or registered students who are required to pay the tuition fees due to the University or the host university of an exchange or attachment program for the period of coverage (for administrative purposes, the period of coverage for Semester One shall be deemed to commence from 1 July 2011).

(2) The Policy shall also cover any matriculated or registered student who incurs medical expenses occasioned by or arising from an Accident that results in his or her being granted leave of absence and hence waiver of tuition fee by the University.

(3) The Policy shall not cover any matriculated or registered student who has not paid the premium collected by the University.   Nevertheless, without prejudice to Item (1) above, the Policy shall cover any matriculated or registered student awaiting the collection of his or her premium by the University in the period of coverage, subject to the subsequent successful collection of the premium.

(4) Coverage for each student being covered shall terminate upon his / her ceasing to be a student of the University.   Coverage for students being covered who are in their final year of study at the University shall lapse when they cease to be students of the University and, without prejudice to Items (1) and (3) above, shall be renewed automatically upon their status as students of the University being reinstated for specific purposes in the period of coverage.

(5) Without prejudice to Items (1) and (3) above, matriculated or registered students who join the University after 1 July 2011 shall be covered from the date he or she officially joins the University.  

Note: For administrative purposes, we regard final-year students who have passed all the examinations to cease being matriculated students of the University on the day after the release of their final-year examination results.

Exclusions

The Policy shall not apply to any of the following:

(a) Routine general physical or any other examinations not directly related to admission, diagnosis, illness or Injury or treatment which is not medically necessary;

(b) Dental care or surgery, cosmetic or plastic surgery except necessitated by Injury caused by Accident;

(c) Congenital anomalies and conditions arising out of or resulting therefrom;

(d) Any mental and/or nervous disorders, sleep disorders, convalescence of rest care, alcoholism and drug-related treatment;

(e) Miscarriage (except Accidental), abortion, pregnancy;

(f) Pre-existing condition or illness;

(g) Self-destruction or any attempt thereat while sane or insane;

(h) Insurrection declared or undeclared war or any warlike operations, military or naval service in time of declared or undeclared war or while under orders for warlike operations or restoration of public order;

(i) Participation in riot, committing an assault or felony;

(j) Participation in competitive racing on wheels;

(k) Motorcycle rider or pillion rider engaging in or practicing for racing and hill climbing contests and reliability trials and speed or duration testing.

Other information

The policy for AY 2011/2012 (period of policy: 1 Jul 11 to 30 Jun 12) is underwritten by Chartis Singapore Insurance Pte Ltd.   The premium for AY 2011/2012 for the General Cover shown above is S$5.35 (including GST) per student.   Eligible students who wish to seek a claim for Accidental Injuries must notify Student Affairs Office by emailing the contact persons shown below under "Disclaimer and contact information".   This email notification must be made within 30 days after the occurrence of the Accident. 

Note on Travel Insurance: Group Personal Accident (GPA) insurance is NOT the same as Travel Insurance (TI).   You should seriously consider purchasing TI on your own if you are travelling overseas and may want to be covered for items such as loss of baggage, trip cancellation, evacuation expenses and repatriation expenses.

Applications related to Industrial Attachment, Enhanced Industrial Attachment, International Research Attachment, Professional Internship and Industrial Internship

Applications are now closed for full-time undergraduates undergoing Industrial Attachment, Enhanced Industrial Attachment, International Research Attachment, Professional Internship and Industrial Internship from Jan 12 to opt for additional cover in addition to the General Cover above.   The list of students whose premium payments for the optional additional cover have been received has been posted at Notice Board (Students) of the Public Folders (please refer to the posting posted in the afternoon of 16 Jan 12).   Any eligible student who has opted for the additional cover and paid the premium but whose name is not listed is advised to email Mrs. Christine Tan at chng@ntu.edu.sg

Those travelling overseas are reminded to purchase sufficient travel insurance separately from the Group Personal Accident Insurance Scheme.
