NTU Library Survey 2019 - Consent Agreement

The purpose of this research is to understand NTU faculty, students and staff’s expectations and perceptions of NTU Library resources and services.

Procedures: You will need to complete an online survey which is expected to take between 10-15 minutes.

Right to Refuse or Withdraw: Your participation in this study is completely voluntary, and you may withdraw from the study at any time without penalty. However, it is important to us that you complete as many questions as possible. Your decision to participate, decline, or withdraw participation will have no effect on your status or relationship with the Nanyang Technological University.

Risks and Discomforts: There are no anticipated risks, beyond those encountered in daily life, associated with participating in this study.

Benefits: You will have an opportunity to give feedback on library resources and services, and be eligible for a lucky draw to win 1 of 7 prizes.

Anonymous and Confidential Data Collection: Your participation in this study will remain confidential, and your identity will only be used for lucky draw and recruitment of focus group participants only.

Confidentiality of records: Your responses will be assigned a code number that is not link to your name or other identifying information. Results of this study may be presented at conferences and/or published in books, journals, and/or in the popular media.

Personal Data: “Personal Data” means data about you which makes you identifiable: (i) from such data; or (ii) from that data and other information which an organisation has or likely to have access. By signing the Consent Form attached, you (or your legally acceptable representative, if relevant) are authorizing collection, access to, use and storage of your “Personal Data". Research arising in the future, based on this “Personal Data”, will be subject to review by the relevant institutional review board.

Data collected are the property of Office of Information, Knowledge and Library Services (OIKLS). In the event of any publication regarding this study, your identity will remain confidential. Any research data containing your “Personal Data” that is collected for the purposes described in this Consent Form will be stored in Singapore.
